Sandy Bertrand Physical Therapy does not participate in insurance plans or filing. A “super bill” is provided with complete information required for self-filing. It is the patient’s responsibility to contact their insurance company for information on filing and reimbursement.

Fees are charged according to services rendered and will vary each appointment. Range of fees: minimum – $96, maximum – $126.
