To schedule, manage or cancel appointments, please visit the Client Portal. Please check your appointment schedule a few days before your expected appointment just in case there is a missed communication. We look forward to seeing you!

COVID POLICY: Please do not come to or schedule an appointment if you have been exposed to a COVID-positive person within 2 weeks, have traveled in the past 2 weeks, or have any symptoms of illness.


Cancellation Payment Policy

Our appointment schedules can be very full. As a result, we understand many patients like to schedule appointments well in advance. Because this can pack our schedules even further, we ask that you cancel appointments as far in advance as possible as well. Due to high numbers of late cancellations and no-shows, a cancellation policy has been instated. Cancellations within 48 hours will be charged a $25 missed appointment fee. No-shows will be charged the full price of the visit. The goal of this policy is to allow us to meet the needs of more patients in a more expedient manner.

Cancellations can be made via the Client Portal 48 hours or more in advance. For cancellations within 48 hours, send an email to: sandybert64@hotmail.com.


Client Portal FAQs

Am I a new or existing client?

New Client

Select the New Client option and follow on-screen instructions if you DO NOT have any upcoming appointments already scheduled.

  • Your Client Portal account has not yet been created.

  • When you request an appointment, an account will be created for you on the Client Portal.

  • You will receive an email with your Client Portal access information when your appointment request is confirmed.

Existing Client

Select the Existing Client option if you ALREADY HAVE an upcoming appointment scheduled.

  • If you already have an upcoming appointment, your Client Portal account has already been created for you.

  • If you do not know your login information, click on Forgot Password to retrieve or create a password.

  • Once you are logged in, you can request new appointments or manage existing appointments.

 
 

How do I sign up to receive text or email appointment reminders?

In order to receive text an email appointment reminders, please ensure that you opt in to these options when filling out your Contact Information form. The Contact Information form is located in the Documents area of the Client Portal.

Once enabled on the Contact Information form, email reminders are sent out 48 hours prior to your appointment, and text reminders are sent out 24 hours prior to your appointment.

How do I update my contact information?

To update your contact information, click on the chat icon in the top right of your Client Portal and send a secure message with the information, or send an email to sandybert64@hotmail.com.

How will I know my appointment has been confirmed?

All appointment requests made through the Client Portal must be manually confirmed by Sandy Bertrand PT. When your appointment is confirmed, you will receive an email stating that it is confirmed. The email will contain a link for you to easily add the appointment to your electronic calendar.

All appointments can be managed through the Client Portal, where you can always view a list of your upcoming appointments.

What documents will be requested when I log into the Client Portal?

You may be requested to fill out the following:

Contact Information
Make sure to fill this out to receive email and text appointment reminders.

Physical Therapy Intake and History Form
This document will be requested for evaluation of new symptoms or injuries.

Informed Consent for Physical Therapy, Notice of Privacy Practices, and Practice Policies
Approval of each of these three documents is requested one time only so that it will be part of your electronic health record.

Credit Card Payment Information
This will allow you to put a card number on file for appointment payments.
